Medical Coordinator
Entry Rate: $18 – 21 per hour (negotiable on experience)Job Type: Full-time
Description
The Medical Coordinator will plan, track, monitor and ensure that all clients’ medical needs are being met within appropriate time frames. Primary responsibilities include; the scheduling of all annual, follow-up and emergency medical appointments. These include communicating with medical professional staff, agency drivers, program specialists, supervisors and program directors. The medical coordinator performs routine clerical and administrative tasks necessary to resolve any issues that may arise, and documents all of their activities in accordance with the agency’s policies. The medical coordinator also needs to engage in staff training and support activities. The medical coordinator need to share information with the residential site and staff regarding any changes within a reasonable period of time.
Under the direction of the Assistant Residential Director, primary responsibility is to schedule all client medical appointments.
– Observes and monitors the client’s status through paperwork and follow-up appointments.
– Observes and tracks clients’ physical care including, but not limited to annual and emergency appointments.
– Provides accurate, timely, and consistent documentation of client medical appointments in the appropriate records.
– Communicates with supervisors, co-workers and medical professionals’ regarding clients’ status.
– Maintains the Access database of client’s medical records.
– Review and report all clients’ medical status changes immediately.
– Maintain a working knowledge of Individual Support Plan, Behavior Support Plan and any other clinical plan developed for the effective support of the individual.
– Assist supervisors on medical appointments as needed
– Attend all pertinent medical appointments that need to be addressed more clearly and precise to achieve the best results for the individual
– Must be able to lift up to 50 pounds.
-Maintain a professional relationship with all providers to ensure the best care for the individuals
– Review all medication and medication logs in the homes for any changes, refills, or errors weekly and report back to the Assistant Director/Program Director
– Process and update necessary information for each individuals medications brought into the office from the pharmacy each month within an appropriate time frame and have them ready to be distributed accordingly by months end
– Act as on call site supervisor monthly
– Other duties as assigned
Qualifications
Education:
– High school or equivalent (Preferred)
License/Certification:
– CNA (Preferred)
– CPR Certification (Preferred)
– Driver’s License (Preferred)
